THIS SERVICES AGREEMENT (this “Agreement”) is made and entered into this <<Insert Date>> (the “Effective Date”), by and between the CHILDREN & FAMILIES COMMISSION OF FRESNO COUNTY, a California public entity established in accordance with the California Children and Families Act of 1998 (the “Commission”), and <<Agency Name>> a California <<Type of Agency from W9 Form>> (“Contractor”).
