This Settlement and Release Agreement (“Agreement” or “Settlement Agreement”) is entered into by and between DuPage Medical Group, Ltd. d/b/a DuPage Medical Group1 (“Defendant”), and Rochelle Hestrup, Erin Peiss, Lindsey Casares, Robert Casares, William Elenitsky, Ken Pardue, Teresa Polanski, and Nicole Sanders as mother and next friend of Rebekah Sanders and Zoe Sanders, minors (“Plaintiffs”), both individually and on behalf of the Settlement Class, in the case of Hestrup et al. v. DuPage Medical Group, Ltd. d/b/a DuPage Medical Group, No. 2021L937 (18th Cir., Illinois), currently pending in the Circuit Court for the Eighteenth Judicial Circuit, DuPage County, Illinois (the “Litigation”). Defendant and Plaintiffs are each referred to as a “Party” and are collectively referred to herein as the “Parties.”
