• It is a requirement that I screen my child for Covid-19 symptoms each school day to ensure they can attend school. I agree that I will monitor my child each day prior to sending them to school. If the answer to any of the following screening questions is yes, I agree that my child will not get on a school bus or be sent to school and that I will promptly contact my health provider and/or the local Public Health Unit, which may result in having my child assessed prior to their return to school.
