This indemnification, hold harmless and waiver of rights agreement, made and entered this (date below) by and between Canine Sports L.L.C. (hereinafter referred to as “First Party”) and (owner name below) (hereinafter referred to as “Second Party”) is in consideration of First Party’s grant of permission to use agility, rally and/or obedience equipment owned and controlled by First Party and Second Party’s use of said agility, rally and/or obedience equipment and other good and valuable consideration received from or on behalf of First Party, receipt of which is acknowledged, it is hereby agreed as follows:
