This contract for the personal services of performers for the engagement described below is between the University of Wisconsin-Madison herein referred to as “University” and the undersigned performer or performers (herein called the “Artist” even though there may be more than one person.) The contract consists of “University of Wisconsin-Madison Entertainment Contract” and the attached “Additional Conditions”. Please print the document and complete all questions or fields marked .
