I acknowledge that the nature of the Course/Activity/Trip may expose me to hazards or risks that may result in my illness, personal injury, or death, and I understand and appreciate the nature of such hazards and risks. I represent that I am physically able, with or without accommodation, to participate in the above-referenced Course/Activity/Trip, am able to use the equipment and/or supplies associated with the Course/Activity/Trip, and have obtained all required immunizations.
