This Task Order to Agreement for Type of Services (“Task Order”) is made as of INSERT DATE, and forms a part of the Agreement for Program Tasks and Services between LONG BEACH UNIFIED SCHOOL DISTRICT (“District”) and INSERT NAME OF COMPANY (“Consultant”) (individually a “Party” and collectively the “Parties”) dated on or about INSERT BOARD APPROVAL DATE (“Master Agreement”). This Task Order incorporates Services to be performed by Consultant as detailed in general work description.
