I/We am in good health. I have not recently been treated for, nor am I aware of any condition that would jeopardize my health or prevent my full participation in the activities that form part of the adventure tour I am participating in. I agree that in the case of an emergency, event staff or a delegate have my permission to request medical treatment for me if I am not in condition to make such decision myself.
