This agreement is made effective upon the receipt of device, between the Tomah Area School District, the student receiving a device and his/her parent(s) or legal guardian. While there are differences between some Tomah Middle and Tomah High School procedures, the information contained within this document applies to both schools. The student and parent(s), in consideration of being provided with a device, software and related materials for use while a student at either the Tomah Middle School or Tomah High School, hereby agree as follows:
