As the adoptive owner of the above named rescued dog, I agree to have this dog neutered or spayed by 6 months of age, unless there is a medical reason with verification from a Veterinarian. If the dog is of age but not altered upon adoption, I agree to have the dog neutered or spayed within 60 days of the adoption date. I understand the rescue will cover the cost of the neuter or spay only if I use a Biggie Paws Rescue approved Veterinarian. If you choose to use your own vet Biggie Paws Rescue will reimburse $100 once we receive vet confirmation or a spay/neuter certificate. I must send a scanned copy of the dated veterinary bill (Please be sure the dog’s name and description are clearly marked on the invoice) that reflects the charges for the neuter or spay to:
