This Surgical Critical Care Fellowship Appointment Agreement (this “Agreement”) is entered into to be effective as of July 1, 2025 by and between Spartanburg Regional Health Services District, Inc., a political subdivision of the State of South Carolina, 101 E. Wood Street, Spartanburg, South Carolina 29303 (the “District”) and <NAME> (the “Fellow”).
