Contract Type FiledAugust 28th, 2020• This shared care agreement (SCA) has been written to ensure and enable the continuation of care by primary care clinicians of patients receiving DMARDs initiated by the Gastroenterology, Rheumatology and Dermatology Specialist Teams for the conditions listed in the product monographs within this document.• The Disease Modifying Anti-rheumatic Drugs (DMARDs) Azathioprine, Ciclosporin, Leflunomide, Mercaptopurine, Methotrexate, Mycophenolate Mofetil, Penicillamine and Sulfasalazine are AMBER on formulary which means they are to be initiated and titrated by secondary care. Once the patient is stabilised on treatment, primary care can agree to the prescribing and monitoring as laid out in this agreement.• ONE signed agreement must be completed for each patient.• The signed agreement must be scanned into GP records and read coded accordingly ……code?
