Low Vision Examination and Low Vision Prescription Services, as outlined in the New York State Commission for the Blind (NYSCB) Low Vision Guidelines, can be performed under contract or as a non-contract provider. Non-contract providers are permitted to provide services as specified within the NYSCB Low Vision Examination and Low Vision Prescription Services Guidelines up to discretionary limits (currently set at $49,999) over the period from first date of service to last date of service during the 5/01/2021 - 4/30/2026 contract terms and will be paid through a Consumer Information System (CIS) authorization*. To become authorized as a non-contract provider please complete this form. Services cannot be performed under the non-contract provider authorization agreement until it has been reviewed and approved by NYSCB.
