This Authorization, Release and Waiver of Liability Agreement (this “Agreement”) executed on the date set forth below, by the person signing this Agreement where indicated below (“Volunteer”) releases Planned PEThood of Georgia, Inc. (hereinafter referred to as “PPHGA”), a non-profit corporation organized and existing under the laws of the State of Georgia, and each of its directors, officers, employees and agents.
