This Agreement sets forth the terms of your membership in Tendwell’s Direct Health Care Program (“Clinical Program”) with Tendwell, LLC, or Tend Functional Health, A Holistic Nursing & Adult Health NP Practice, PLLC (individually a “Tendwell Practice” and collectively “Tendwell”). The Clinical Program is designed to provide you with direct personalized medical services.
