THIS AGREEMENT made and entered by and between the Public Health Trust, an agency and instrumentality of Miami-Dade County, Florida (hereinafter referred to as the "Trust", “Owner”, or “JHS”), which operates Jackson Health System, including Jackson Memorial Hospital, located at 1611 Northwest 12th Avenue, Miami, Florida 33136, and other designated health care facilities, and TBD, FEI Number TBD, a corporation organized and existing under the laws of the State of TBD and authorized to do business in the State of Florida (hereinafter referred to as the "Consultant").
