If you are a contracted consultant, a vendor, a physician, a laboratory or other entity whose relationship requires access to hospital information or who has been permitted access to patient identifiable information (PHI), you will also acknowledge the terms of the Griffin Hospital HIPAA Confidentiality Statement and Agreement.
