THIS AGREEMENT (the “Agreement”) is made this day of , 20 (the “Effective Date”) by and between The University of Chicago Hospitals ("UCH") and (“Third Party”). UCH and Third Party will each individually be referred to as a “Party,” and collectively as the “Parties.”
