This form assigns primary responsibility of one (1) RC Channel Transmitter from the Rogers State University to the borrower. The borrower will be responsible for taking the necessary precautions to protect the equipment and store it in a manner that provides adequate protection, when it is not in use, thus not subjecting the equipment to damage or possible theft. The borrower will be responsible for returning the equipment in its original condition. If it is determined that damage has occurred as a result of negligence, the borrower will be financially responsible for the replacement of the equipment.
