This Amendment is made part of the Administrative Services Agreement and is effective August 1, 2019. This Amendment supplements and amends the Agreement between Employer and HealthKeepers, Inc. and Anthem Health Plans of Virginia, Inc. dba Anthem Blue Cross and Blue Shield. If there are any inconsistencies between the terms of the Agreement or its Schedules and this Amendment, the terms of this Amendment shall control.
