This Agreement is made and entered into this 5th Day of February, 2013, by and between Archuleta County (“County”), through the Sheriff’s Office (“Sheriff”), and Jay Mathers, MD (“ Contractor”), collectively referred to herein as the “Parties”, for the administration of their respective duties concerning physician oversight for personnel providing emergency medical services in the pre-hospital environment under the direction and control of the Sheriff.
