THIS AGREEMENT, dated as of is entered into by and between (hereinafter referred to as “Resident/Fellow Physician”) and the University Health Center of Pittsburgh dba University of Pittsburgh Medical Center Medical Education Program (UPMCMEP), a non-profit corporation organized and existing under the laws of the Commonwealth of Pennsylvania (hereinafter referred to as (“UPMCMEP”) which is a cooperative venture of various hospitals that are part of or affiliated with the University of Pittsburgh Medical Center system with approved residency-training programs and with the principal offices at 3708 Fifth Avenue, Pittsburgh, Pennsylvania 15213.
