This Subscription Amendment Agreement (“Agreement”) is being entered into between Ellevation Inc., having an address at 38 Chauncy Street, Boston, Massachusetts 02111 (“Ellevation”) and the Company Name identified as “Customer” below. Customer understands that this Agreement is governed by the Terms and Conditions agreed upon in the most recent signed contract, which are incorporated herein by reference. In the event of any conflict between this Agreement and and the most recent signed contract, the terms of this Agreement shall control. This Agreement is effective as of the Subscription Start Date set forth below.
