THIS COLLABORATIVE DRUG THERAPY MANAGEMENT AGREEMENT (this “Agreement”), made this 5th day of September, 2017 (the “Effective Date”) between the physicians executing this Agreement (each, a “Physician” and collectively, “Physicians”), and the pharmacists executing this Agreement (each, a “Pharmacist” and collectively, “Pharmacists”).
