This Agreement to provide Claims Administration for the Self-Insured County of Orange Dental Plan, hereinafter referred to as “Contract”, is made and entered into as of the date fully executed by and between the County of Orange, a political subdivision of the State of California; hereinafter referred to as “County,” and California Physicians' Service, dba Blue Shield of California with a place of business at 601 12th Street, Oakland, CA 94607; hereinafter referred to as “Contractor,” with County and Contractor sometimes referred to as “Party”, or collectively as “Parties.”
