This Weight Management Consulting and Personal Training Services Agreement (this “Agreement”) is by and between Client and Cindy Giles d/b/a Restoration Wellness (“Trainer”), effective as of the date above written. This Agreement provides a description of Trainer’s offered services, the associated fees, and other important terms of our relationship.
