By this Agreement, it is my intention to forever relieve JEFFREY TEDDER d.b.a. Half Notes ECM, its officers, employees, and agents, volunteers, (hereinafter “Mr. Tedder”) of any duty to me or participant and I do assume the entire risk of any personal injury, damage, loss, harm, claim, medical expense and any other type of expense that might occur as a result of participation in music classes. By this Agreement, I also intend to waive and relinquish any claim or cause of action against Mr. Tedder for any personal injury, damage, loss, harm, medical expense and any other type of expense caused to PARTCIPANT and promise not to sue or exercise any legal right to seek damages from MR. TEDDER.
