The therapeutic relationship is unique in that it is a highly personal and at the same time, a contractual agreement. This agreement discusses what you can expect in therapy as well as your rights and privacy protections in therapy. Your signature indicates that you agree to the stipulations set forth in this document and that you have received a copy of my HIPAA/Privacy Policy, which is always available for review.
