This Participant Agreement (“Agreement”) is between KentuckyOne Health Partners, LLC, a Kentucky limited liability corporation (“KYOne HP”), and an entity organized as a nonprofit corporation under Kentucky law (“Participant,” “You” or “Your”) on its own behalf and with respect to each “Provider/Supplier” affiliated with Participant which has executed a Provider Supplier Addendum attached hereto as Exhibit B.
