Pursuant to the Continuing Disclosure Agreement, dated February 5, 2015, between Lakeland Regional Health Systems, Inc. and Lakeland Regional Medical Center, Inc. (collectively, the Health System) and Digital Assurance Certification, LLC (DAC) in connection with the $180,000,000 Series 2015 Hospital Revenue Bonds; the Continuing Disclosure Agreement, dated July 28, 2011, between the Health System and DAC in connection with the $77,580,000 Series 2011 Hospital Revenue Refunding Bonds; and the Continuing Disclosure Agreement, dated November 2, 2006, between the Health System and DAC in connection with the $113,005,000 Series 2006 Hospital Revenue Refunding Bonds, we are disclosing the signing of the sixth amendment to the Lease and Transfer Agreement (the Agreement) between the City of Lakeland (the City) and the Health System on August 6, 2015, effective September 30, 2015.
