Please attach your post-dated cheques and non-refundable registration fee to this sheet. Please submit 10 post-dated cheques for monthly fees beginning September 1st 2015. Please be sure to date 2016 cheques correctly. Cheques should be made payable to the Chelsea Cooperative Nursery School (the abbreviation CCNS may also be used). Parents must notify the registrar at least one full calendar month in advance if their child is to be withdrawn from the program so that a replacement family can be found. Payment in full for the calendar month following notice of withdrawaL is required.
