(“BF” or “our firm”), a Registered Investment Adviser acting as a 3(21) Fiduciary, whose principal place of business is 4629 Burnet Avenue, Sherman Oaks, CA, 91403 , and the designated ERISA Plan (“Plan”) (collectively “Parties”), qualified under the Internal Revenue Code, as amended. The Plan is subject to the Employee Retirement Income Security Act of 1974 (“ERISA”).
