For good and valuable consideration, the receipt of which is acknowledged, I hereby grant to Stony Brook University, and its Affiliated Entities including but not limited to, Stony Brook Foundation, and the Faculty and Student Association (the “University”), a non-exclusive, [perpetual license] or [ year] to use, distribute and create derivative works of the material known as (“S-BOLD Materials”), as more fully described below. This license includes rights to internal use for educational purposes and includes authorization to use my name, voice likeness, biographic information and supporting material in connection with the S-BOLD Materials.
