This membership is a one-year membership with payments made monthly with a one- time initiation fee of $100. All members agree to provide their credit card information to be kept on file and the card on file will be charged on the first of each month. All members agree to abide by all rules, regulations, and policies of Belmont. I understand and agree that the membership will begin on the date the form is completed and the initiation fee is paid. The term of the membership will end one year from that date. We understand that in the event of an infringement of this Agreement, Belmont reserves the option to revoke my membership and all rights and privileges of Belmont with no refund of any amount previously paid. Cancellation of this membership before the 12 month agreement is subject two-month cancelation fee and must be made in writing.
