INSTRUCTION AGREEMENTInstruction Agreement • April 28th, 2013
The lesson fee is $100.00 per month. This amount is for one 30 minute lesson per week and will be the same amount every month regardless of any student vacations, cancellations or holiday’s. There will be no make-up lessons, refunds, or monitory adjustments. The student will pay for one month of lessons in advance to reserve their scheduled lesson time. The monthly lesson fee will be due on the first lesson of each month. If the lesson fee is not received on the first lesson of the month, you will be required to mail the payment to the address above so that the payment is received before the second lesson of the month. If your teacher needs to cancel a lesson, a make- up lesson will be given. If the student wishes to cancel instruction, a two weeks notice must be given. Please have the courtesy to show-up on time to your lesson since it will not extend past your scheduled lesson time, in fairness to the next incoming student.
