It is the parents’ responsibility to read this contract completely before signing. It is the parents’ responsibility to abide by all contractual agreements as stipulated in this contract. This is a legal and binding contract. The contents of this contract will cover fees, payment of fees, late fees, vacations, holidays, hours of service, termination policies, illnesses, child illnesses, liability, behavior problems, medications, and other miscellaneous Items. Before daycare services can be rendered this contract must be signed in all required areas, completely filled out as required, returned to the provider along with all registration forms and other forms required by the provider. All forms must be submitted before child can attend daycare. Shelley Radcliff herein called “Provider”, agrees to provide daycare for (First and Last name).
