Completion of this form is required by all supervisors and all students at the beginning of their program. Conditions of supervision must be reviewed annually. Any revisions to this agreement must be submitted in a revised and signed statement from the student and supervisor to the Director of Doctoral Programs. Signatures of student and supervisor indicate their intent to abide by the terms of this agreement.
