This Fitness Passport Service Agreement (this “Agreement”) is entered into on Agreement Start Date (Domestic) (the “Effective Date”), by and between Optum Entity, including its affiliates (“Optum”) and Primary Counterparty (”CrossFit”). For purposes of this Agreement, CrossFit and Optum shall collectively be referred to herein as the “Parties” and individually as a “Party”.
