This Agreement is entered into by and between the Washington State Pollution Liability Insurance Agency (PLIA) and (Service Provider). The purpose of this Agreement is to provide instructions to the Service Provider and to outline the terms and conditions that the Service Provider must comply with when engaged in the performance of work under PLIA’s Heating Oil Insurance Program.
