This agreement confirms arrangements for a graduate level internship placement for (YOUR) NAME ________________________ at (YOUR AGENCY) _________________________ during the SEMESTER of ___________YEAR ______. The semester begins (SEMESTER BEGIN Date) _______________ and concludes (SEMESTER END DATE) _________________. Students must complete a minimum of 480 hours (typically three months minimum at an average of 40 hours per week) and remain in the Agency throughout the duration of the semester.
