THIS NURSING FACILITY SERVICES AGREEMENT ("Agreement") is made and entered into this 20th day of February , 2024 (the "Effective Date") by and between Addus Hospice of Illinois, LLC dba JourneyCare ("Hospice") and DeKalb County Rehab & Nursing Center ("Facility").
