This document sets forth the terms pursuant to which I, , have agreed voluntarily to denote some of the eggs (called "oocytes") which I will produce during my menstrual cycle to one or more women (each called a "Recipient") who have been unable to have children and who wish to attempt to have children through the treatment process referred to as in vitro fertilization ("IVF"). The egg donation process process will be undertaken through the Vincent Reproductive Medicine & IVF Program, Massachusetts General Hospital. Said program, including respective employees, agents and assigns shall be referred to in this document as the "IVF Program".
