This Software-as-a-Service Agreement (this “Agreement”) is between CyberGRX, Inc., a Delaware corporation, with its primary place of business at 1637 Wazee Street, Suite 400, Denver, CO 80202 (“CyberGRX”) and [INSERT CONTRACTING ENTITY], a [INSERT STATE OF INCORPORATION] [INSERT ENTITY TYPE], with its primary place of business at [INSERT ADDRESS] (“Customer”), and is effective as of [ , 20 ] (the “Effective Date”). Each of CyberGRX and Customer may be referred to herein individually as a “Party” or collectively as the “Parties.” This Agreement allows Customer to enter into orders placed under this Agreement (each, an “Order”) for Services (as defined below) provided by CyberGRX under the terms of this Agreement.
