This ONSITE SERVICES AGREEMENT (the “Agreement”), dated as of the Effective Date appearing below, is made and entered into by and between Catapult Health, LLC (“Catapult”) and Comal County, Texas, a political subdivision of the State of Texas, as plan administrator acting on behalf of its group health plan (“Customer”) and sets forth the terms and conditions under which Catapult will provide the initial and periodic on-site preventive health and related services (the “Services”) to Customer’s employees, adult dependents and others designated by Customer, as applicable (“Participants”). This Agreement includes, and is subject to, the General Terms and Conditions, Exhibits and Scheduling Addendum, attached hereto.
