This Software and Service Agreement ("Agreement") is entered into on DATE HERE between Mutare, Inc., 2325 Hicks Road, Rolling Meadows, Illinois, 60008, hereinafter referred to as “Provider” and CUSTOMER, CUSTOMER ADDRESS and its affiliates, hereinafter referred to as "Customer”.
