The provider entities presented in the table appended below are included as part of the Participation Agreement signed by the Parent Organization below as subsidiaries, affiliates or locations, in accordance with Exhibit A, Definition v. Each entity is individually entitled to the rights and subject to the obligations of the Participation Agreement. Parent Organization represents and warrants that it has legal authority to bind each entity listed below to the Participation Agreement and that it will provide the CRISP Designated Contact, as set forth on the signature page of the Participation Agreement, with advance written notice of any additions, modifications or deletions of the entities identified below.
