This Amendment (“Amendment No. 1”) to Contract No. MA-042-20012190 for Medi-Cal Administrative Activities is made and entered into on July 1, 2023 (“Effective Date”) between the COUNTY OF ORANGE, a political subdivision of the State of California (COUNTY) ), through its Health Care Agency, with a place of business at 405 W. 5th St., Ste. 600, Santa Ana, CA 92701, and <<CONTRACTOR>> (“Contractor”), a [California (nonprofit corporation) (for-profit corporation) (limited partnership)] with a place of business at <<CONTRACTOR LEGAL ADDRESS>>. The Original Contract and this first Amendment are and shall continue to be administered by the Director of the COUNTY’s Health Care Agency or a Deputized designee (“ADMINISTRATOR”). CONTRACTOR and COUNTY may sometimes be referred to individually as “Party” or collectively as “Parties”.
