I, the undersigned student, acknowledge that I have attended a laboratory safety orientation. I have read and agree to abide by the policies and procedures outlined in the "Laboratory Safety Agreement and Procedures". I further agree to follow the directions given by my laboratory instructor and my laboratory manual regarding the safe use and proper disposal of all materials, as well as the safe and proper use of all equipment. I understand that failure to abide by the aforementioned policies, procedures, and directions shall lead to sanctions as outlined in the letter from the Dean of the College of Arts and Sciences.
