City to apply dust control to the gravel road abutting their property in accordance with the City’s adopted Dust Control Policy. Pursuant to that policy they hereby submit payment for the requested dust control in the amount of $250.00 for 500 feet of dust control application. In the event that, for whatever reason, payment is dishonored, applicant agrees to have said payment assessed against the described property and waives any and all hearings or rights of appeal, statutory or otherwise.
