This Business Associate Agreement (this “Agreement”) is entered into effective as of [3], by and among [4] Revolution Health & Wellness and [5] (herein “Business Associate”) in order to comply with 45 C.F.R. §164.502(e) and §164.504(e), governing protected health information (“PHI”) and business associates under the Health Insurance Portability and Accountability Act of 1996 (P.L. 104-191), 42 U.S.C. Section 1320d, et. seq., and regulations promulgated thereunder, as amended from time to time (statute and regulations hereafter collectively referred to as “HIPAA”) [Covered Entity and Business Associate may be referred to herein individually as a “Party” or collectively as the “Parties”].
